What We Will Not Do
We will not provide your personally identifiable responses to any market research survey question or educational program request for information to any third party, other than as expressly set forth below, unless such question or request is expressly identified. For instance, we may ask you at the end of an educational program if you wish to receive additional information on the topic presented or samples of a drug mentioned. Typically, we would forward such requests for information to a third party, such as a pharmaceutical company, for fulfillment.
We may provide your personally identifiable information to companies that provide services to help us with our business activities such as mailing and verifying medical education numbers/DEA numbers. These companies are authorized to use your personally identifiable information only as necessary to provide these services to us.
ImpactNetwork will comply with all applicable laws, such as comply with a subpoena, bankruptcy proceedings or similar legal process, rules, regulations, and orders of any court or government administrative body. Such compliance may require the disclosure of your personally identifiable information, responses and requests. We may also disclose personally identifiable information when we believe in good faith that disclosure is necessary to protect our rights, protect your safety or the safety of others or investigate fraud. ImpactNetwork will use reasonable efforts, as appropriate, to avoid or minimize any public disclosure.
The Information We Collect
Personally Identifiable Information: We may collect a variety of personally identifiable information, including but not limited to: name, email address, mailing address, medical degree, practice description, DEA number, state license number and medical specialties. We do not collect geo location data through our mobile app. When you download and use our services, we automatically collect information on the type of device you use, operating system version, and the device identifier (or "UDID"). We may send you push notifications through our mobile app to send you service related notifications that may be of importance to you. You may at any time opt-out from receiving these types of communications by turning them off at the device level through your settings.
Survey and Other Responses: Should you choose to participate in market research surveys or educational programs, you will be asked questions, and your responses will be recorded.
Aggregated Demographic Data: Should you choose to participate in market research surveys or educational presentations, we may collect information on your practice characteristics, diagnostic methods, treatment methods, use of and attitudes towards pharmaceutical products, your managed care affiliations, and other standard research data necessary to categorize your practice and report your opinions. This data is collected, analyzed and protected in accordance with standards established by CASRO (Council of American Survey Research Organizations).
How We Use Information
Personally Identifiable Information: The personally identifiable information you provide during registration allows us to verify your medical credentials for entry into the ImpactNetwork web site, identify you as a member of ImpactNetwork, and to customize your ImpactNetwork user experience. Submission of registration data is mandatory to allow use of this web site.
Your registration information is also used to establish an address for mailing of honoraria or should you elect to receive products or services; similarly, your email address is collected to notify you of programs or services you have requested. Other registration elements are used to categorize your specialty and practice for market research purposes and for selection of survey panels and education program audiences. You can elect to opt out of email notification at any time by sending an email to firstname.lastname@example.org with subject line "NOEMAIL" and you can review and modify your personally identifiable data through the "My Profile" button on the web site.
Survey and Other Responses: ImpactNetwork uses the information you provide in response to market research survey questions and educational program requests for information to provide answers to questions, generate hypotheses, and test hypotheses. Except as otherwise explicitly noted, the actual responses you provide in connection with surveys or other programs are de-identified prior to provision to our clients. We may aggregate your responses with those of others.
ImpactNetwork may use a third party service provider to obtain non-personally identifiable information and add that data to our measurements. We may also associate the anonymous unique identifier within the persistent cookie with information obtained from other sources (such as consumer preference reporting companies) using confidential matching procedures.
Other Uses: We never use or share the personally identifiable information provided to us online in ways unrelated to the ones described above without also providing you an opportunity to opt-out or otherwise prohibit such unrelated uses.
If you choose to use our referral service to tell another physician about our site, we will ask you for the name and email address of your referral. We will automatically send this physician a one-time email inviting them to visit the site. ImpactNetwork stores this information for the sole purpose of sending this one-time email and tracking the success of our referral program. Your referral may contact us at email@example.com to request that we remove this information from our database.
Sharing Information with Third Parties (applies only to hygienist and pharmacists ): Solely with respect to hygienists and pharmacists, we may occasionally share demographic or contact information with selected third parties for marketing or promotional purposes. If you would like to opt out from such information sharing, simply send an e-mail to firstname.lastname@example.org .
Tracking Technologies / Cookies
Log Files: When you logon to the ImpactNetwork web sites, we gather certain information automatically. This information may include Internet protocol (IP) addresses, browser type, internet service provider (ISP), referring/exit pages, the files viewed on our site (e.g., HTML pages, graphics, etc.), operating system, date/time stamp, and/or clickstream data.
Mobile Analytics: We use mobile analytics software to allow us to better understand the functionality of our mobile software on your phone. This software may record information such as how often you use the application, the events that occur within the application, aggregated usage, performance data, and where the application was downloaded from. We do not link the information we store within the analytics software to any personally identifiable information you submit within the mobile app.

The security of your personally identifiable information is important to us. When you enter sensitive information (such as a Tax ID), we encrypt the transmission of that information using secure socket layer technology (SSL).
We follow generally accepted standards to protect the personally identifiable information submitted to us, both during transmission and once we receive it. No method of transmission over the Internet, or method of electronic storage, is 100% secure, however. Therefore, we cannot guarantee its absolute security. Our Commitment to Children's Privacy
The myimpactnetwork.com website is not intended for use by minors under the age of 13 years.
ImpactNetwork's ability to protect your privacy also depends on you maintaining your username and password in confidence. Do not share your username and password with anyone else or otherwise permit anyone to access the ImpactNetwork web site using your personal or professional information.
How to Access or Correct Your Information
Upon request ImpactNetwork will provide you with information about whether we hold any of your personal information. You may review and modify your personally identifiable information at any time by selecting the "My Profile" option on the "Welcome" screen. Should you wish to delete your personally identifiable information or disenroll from the ImpactNetwork service at any time, send an email to: email@example.com with subject line "DISENROLL". We shall respond to any requests for access to personally identifiable information within 30 days.
Opt-out Preferences: When you register for an account, we will use your name and email address to send periodic emails to you of both promotional and transactional in nature. Out of respect for your privacy, you may choose to stop receiving promotional emails by following the unsubscribe instructions included in these emails or you can contact us at firstname.lastname@example.org.
We will also send you service related email announcements on rare occasions when it is necessary to do so. For instance, if our service is temporarily suspended for maintenance, we might send you an email. You do not have an option to opt out of these emails, which are not promotional in nature.
