What kind of information do I need
to provide at registration?

Our simple step-by-step registration process requires the following information:

  • Name
  • Valid form for authenticating used your status as a physician or other medical professional.
    • Impact Research issued user name
    • DEA Number
    • Medical Education Number
    • Impact Research issued registration code (used for medical professionals who do not have a DEA or ME number)
  • Primary Office Address
  • Honoraria Mailing Address (optional)
  • Office Phone and Fax Numbers
  • A brief description of your practice type
  • Your Specialties

You will then select your personal Username and private Password and select a Hint question (to assist us if you ever lose or forget your password).
