to provide at registration?
Our simple step-by-step registration process requires the following information:
- Name
- Valid form for authenticating used your status as a physician or other medical professional.
- Impact Research issued user name
- DEA Number
- Medical Education Number
- Impact Research issued registration code (used for medical professionals who do not have a DEA or ME number)
- Primary Office Address
- Honoraria Mailing Address (optional)
- Office Phone and Fax Numbers
- A brief description of your practice type
- Your Specialties
You will then select your personal Username and private Password and select a Hint question (to assist us if you ever lose or forget your password).
